Privacy Statement
The business name of my practice is Curious & Brave Ltd.
Introduction
One of the things that helps make coaching / psychotherapy effective is that it offers you a contained and private space within which you (both) can gradually unfold and explore your life, challenges, dreams and potential. I seek to create and maintain a safe private space for this to occur. Our conversations are private. The GDPR seeks to protect your privacy in respect of data held on you and in particular sensitive data. I have thought carefully about your privacy and designed systems which seek to support that ethos. The GDPR asks for specific consent to be given for data to be processed, and so I am asking you to sign and confirm that consent.
Limits to your confidentiality
In common with all psychotherapists I discuss my work with a trusted and experienced supervising therapist on a regular basis - in a structured process of consultation. The purpose of this is to develop and strengthen my capacity to work with you (both). The conversations are governed by a strong ethic of confidentiality and respect. If I discuss my work with you in such a forum, your identity is protected in order to preserve your privacy through this reflective process.
I do not disclose notes or the content of sessions to third parties unless required to do so as part of a legal process. If that were the case I would seek to discuss this in detail with you first. If you or another person is at serious risk of harm I may wish to speak to your GP or other persons who can assist in such a situation. I would never take this step lightly and would always discuss it with you in detail first as far as possible.
What records do I keep and how are they kept?
I have the records of your name and contact details, and the information which you gave at your initial assessment (‘the assessment records’). These are kept in a locked box at my home.
I may keep some brief notes of our sessions to act as an aide memoire as our work together progresses, for supervision. These notes are hand written and do not identify you personally - if I refer to clients I use their client code or a generic ‘C’ (client). My current notebook is kept in a locked cabinet or in my work bag. Any finished notebooks are kept in a locked drawer separate from assessment records.
I keep your contact details on my phone for which I maintain a secure password. My email is also accessible only by secure password.
I use this information to contact you regarding appointments, and I will not pass this information on to anyone else, with the exception of my clinical will. My clinical will is a password protected document which contains your contact details, that is made available to my supervisor and a trusted colleague in the event of my incapacitation or death. This is to inform you and guide you through any onward referrals you may request. I do not use your contact details for marketing purposes.
All data kept about you including all notes is disposed of carefully seven years after the end of therapy.
If you wish to withdraw your consent to me using your data at any point please let me know by email at katherine@curiousandbrave.co.uk.
Please review the privacy statement above and complete the form below to confirm your agreement.
